Healthcare Access Marylands (HCAM) MATCH Program works with the Baltimore City Department of Social Services to ensure all children in foster care receive health services and their health needs are met. The Registered Nurse Medical Case Manager provides case management for the MATCH program by collaborating with foster parents, parents, and all other members of the interdisciplinary team to coordinate holistic healthcare for children in the MATCH program. The primary responsibilities of this position are: Required to complete responsibilities in a Hybrid work environment. Hybrid is defined as a flexible work model that supports a blend of in the physical office (1-2 days per week) and partially remote workplace.
