Southeast Healthcare is seeking a part-time Family Nurse Practitioner to work two days/week in our Delaware, Ohio office. The FNP will assess the health status of patients, diagnose, plan therapeutic interventions and evaluate the effectiveness of the intervention with the patient. The Nurse Practitioner also determines when consultation and referral is necessary and will consult and collaborate with Southeast staff regarding clients' general health issues, trends, and integration of healthcare.

Requirements

Ohio CNP and DEA required.
