Job Title: Clinical Case Manager

Program: ICMS (Integrated Case Management Services)

Location: 1191 Lakewood Road, Toms River, NJ

Position Type: Full Time

BONUS $2,000.00

Quarterly installments of $500 each beginning after 90 days of employment.

Job Description/Summary

The Clinical Case Manager in ICMS is responsible for providing community-based outreach to link or relink clients to services needed and provide support through recovery goals that prevent risk of hospitalization or re-hospitalization. The Clinical Case Manager assists clients with linkages to community based services to maximize access to services and support clients' recovery.

Responsibilities
Facilitating access to appropriate services and community mental health and non-mental health systems shall be encouraged by ongoing individual clinical support and monitoring.
Coordination, integration, and monitoring of services delivered from multiple providers until consumer is discharged from ICMS are essential to the advocacy process.
Further duties include assisting consumers with accessing transportation, securing appropriate housing, and exploring employment or other meaningful activities.
Demonstrates ability to work on a team and ability to work with other disciplines in a team environment.

Demonstrates ability to telework and work independently with remote support at times.
Demonstrates ability to perform basic computer functions and use of electronic records as required by job function. Proficiently communicates in documentation and verbally

Qualifications
  • Education - Bachelor's Degree in Social Work, Psychology or related field, or AA Degree and four years' experience working with individuals with mental illness
  • Experience - One year post graduation working with individuals with mental illness, preferably adults
  • License - Valid driver's license in good standing.
