Pediatric (Child Welfare) Specialist

(note: Field Based work- More people looking to work within Child welfare- foster care/mommy baby group home/ children's group home)


Position Purpose

Walk me through the day to day responsibilities of this the role and a description of the project (Outside of Workday JD):

  • This position will be field based and will provide care management to assist with coordination of care, including assessing for health care needs, referring to health plan providers, providing education on benefits/expanded benefits, and provide community resources to address social determinants of health needs. The caseload is 100 members. This position will document in an electronic health record and will be required to document outreach notes, complete assessments, and complete a care plan on each member.

Describe the performance expectations/metrics for this individual and their team:

  • Compliance with care management policies/procedures including outreach and documentation expectations.
  • Individuals will be audited on a monthly basis to ensure charts are in compliance.

Tell me about what their first day looks like:

  • The first day will align with training/onboarding. The first day will consist of setting up computer, completing onboarding tasks, reviewing training schedule, and meeting with the training team. Onboarding is 4 weeks.

Required:

Pediatric case management experience

Experience connecting families/children to local resources for various services including employment, housing, independent living, justice, foster care, etc.


Preferred Skills

Health plan experience

Foster care and/or adoption experience


Education Requirement:

Bachelor s Degree


Software Skills Required:

Microsoft Office- Outlook, Word, and Excel
