About the Consortium The West Philadelphia Community Mental Health Consortium, Inc. (Consortium) is a nonprofit (501(c)(3) Community Mental Health Center (CMHC) established in 1967 as one of Philadelphias twelve CMHCs. Throughout our history, we have pioneered providing behavioral health services with compassion first priorities coupled with excellence in mental health treatments and collaborative understanding. For over five decades, we have helped more than 250,000 people throughout the tri-state area overcome the challenges of mental illness and substance abuse. We have been the home of many firsts, from the first methadone clinic to the first clinic providing integrated care. We have a proud and diverse history that is driven by our community. Located in West Philadelphia, the Consortium is the premier provider of Supports Coordination, Outpatient Mental Health, Addictions, Case Management, Eco-systemic Family Therapy (ESFT) and Functional Family Therapy (FFT) for all ages in West and Southwest Philadelphia, serving approximately 4,000 individuals in 2022. Utilizing Evidence Based Practices (EBP) in all programs, the Consortium is accredited by CARF for outpatient drug and alcohol services and has received the EBP Designation through Community Behavioral Healths Evidence-based Practice and Innovation Center (EPIC) for both Cognitive Behavioral Therapy (CBT) and Prolonged Exposure (PE) for Post-Traumatic Stress Disorder (PTSD). Most recently, the Consortiums Integrated Care Program was recognized as Most Innovative Program by the City of Philadelphia. What is Expected in this ROLE This Mobile Crisis Role requires 90% local area travel in a company vehicle. PART TIME for Saturday and Sunday, 7 am-3 pm, 3 pm-11 pm and 11 pm-7 am. Office is based out of location at 5501 Chestnut Street Philadelphia, Pa. The Mobile Crisis Intervention Team provides meaningful counseling, helps individuals in need to develop coping strategies, and facilitate linkage to other agencies or community resources that can be of assistance to the members. Will work with members in their home and in the community. This is not a hybrid or remote position and will be scheduled on some holidays. Provide immediate access to hands-on crisis support through phone counseling and mobile in-person crisis intervention. Provide rapid and effective assessment, support, 302 & 201 hospitalization (when necessary), linkage to community resources, crisis prevention planning, and follow up. Provide all appropriate documentation set forth by Supervisor including intakes, progress notes, medical charting, discharge plans, and individual assessments. Develop and write action plans in a timely manner with clear and measurable outcomes. Participate in assertive outreach to individuals identified to be high users of crisis services. Develop and secure appropriate services for individuals in crisis, including referrals to voluntary inpatient settings and applications for involuntary emergency examination and treatment under 302 of the Mental Health Procedures Act of 1976. Provides linkages to community supports, supportive counseling, and substance abuse services and always reflect the possibility of recovery. Complete referrals for follow-up services by community mental health and other supports. Interface with families of persons served and other interested parties. Maintain communication with physicians, psychiatrist, pharmacist, and other appropriate ancillary service providers. Intervene in crisis situations. Attending scheduled staff team meetings and appropriate in-service training as directed by supervisor. To be available for any of the required shifts; Saturday and Sunday, 7 am-3 pm, 3 pm-11 pm and 11 pm-7 am. Required: A masters degree in human services, or related field, from an accredited college or university with at least one year of crisis experience and at least one year of clinical experience providing assessments and diagnosis of mental illness; history working with individuals who have substance use dependency preferred. Must demonstrate competence in assertive aggression and management of conflict resolution. CPR and First Aid certified. Must provide certificates. Requires up to 90% travel. Strong communication and interpersonal skills. Ability to work collaboratively in a team environment. Excellent organizational skills and attention to detail. Flexibility to adapt to changing needs and priorities.
