Job Summary
HCAM offers diverse connection and coordination services for individuals and families across Maryland through various programs. HCAM's Care Coordination Program (CCP) provides short-term care coordination services to Baltimore City residents who are eligible for Medicaid benefits and need help accessing services. The Care Coordination Program strives to improve birth outcomes and protect our city's most vulnerable populations in Baltimore City though outreach strategies and providing access to health services and community resources by:
Assisting Medicaid/HealthChoice eligible beneficiaries in accessing, and maximizing their Medicaid benefits
Educating clients about health benefits
Linking residents to primary care and specialty services
The Care Coordination Associate will serve as a part of CCP's ACCU team by providing consumers with education, support and additional resources regarding Medicaid benefits and system navigation, assist with addressing barriers to healthcare and act as a liaison to between consumers and HealthChoice Managed Care Organizations, Primary Care Providers, Specialty care and other members of the interdisciplinary team.
Duties And Responsibilities:
Provide outreach and coordination services to Medicaid benefited services including primary care, dental care, specialty care, preventive health screenings, lead screenings, case management, and vaccinations.
Conduct assessment, triage and referral of pregnant women and infants to Baltimore City MCH Programs including WIC, Home Visiting, Parenting Support, Doula services, and Safe Sleep resources.
Provide education and resources to low-cost/ no-cost health care services in Baltimore City, prenatal care, newborn care, EPSDT guidelines, chronic disease management, preventive health screenings, dental care, behavioral health services and other health related services.
Act as a liaison between consumers and HealthChoice Managed Care Organizations, Primary Care Providers, Specialty care and other members of the interdisciplinary team.
Address consumer barriers to healthcare including housing, addiction treatment, domestic violence services, Energy Assistance, emergency funds, Medical Assistance Transportation.
Participates in interagency, external and program related community and intergovernmental engagement meetings, trainings, and outreach events.
Facilitates communication and collaboration between health providers, other program related stakeholders and partnerships.
Collaborates with interagency programs and stakeholders.
Collect, analyze, and distribute program related data and reports to appropriate team members and other key stake holders.
Meet program/agency deadlines and maintain multiple databases.
Ensure accuracy and integrity of utilization & data input of multiple databases.

Qualifications
Minimum Qualifications
Bachelor's Degree in a related area preferred.
Experience providing services to pregnant women and/or infants, adults with chronic health conditions in a public health/community setting- preferred
Excellent planning, time management and decision-making skills
Proficiency in Microsoft Suite, Windows, and other basic computer functions
Excellent written and verbal communication skills
