OverviewThe Clinical Social Worker is responsible for providing developmentally appropriate, outcome-focused diagnostic and therapeutic service to patients and families in accordance with the scope of practice as defined in Chapter 4757 of the Ohio Revised Code, Section 4757.01 et seq. The Clinical Social Worker functions as a member of the interdisciplinary team and provides clinical social work services to patients and their families to improve or maintain social, emotional, functional and physical health: provides consultation, education, and training to staff regarding psychosocial issues that impact the delivery of optimal health care services; and participates in the development and delivery of education, research and continuous quality improvement.Demonstrates competencies defined (*) in Clinical Social Worker responsibilities in providing age-appropriate care to patients from the age of birth to 21 years, and some select adult patients.Why Nationwide Children's Hospital?The moment you walk through our doors, you feel it. A Place to Be Proud.ResponsibilitiesThe Clinical Social Worker implements and supports the philosophy, mission, values, standards, policies and procedures of Nationwide Children?s Hospital and the Patient Care Services Division.The Clinical Social Worker performs case management core functions which include:a. Screening and identification of patientsb. Establishment of an effective relationship with patient/familyc. Assessment of patient/family strengths and needsd. Development of a family-centered plan of caree. Procurement and coordination of servicesf. Monitoring and evaluation of progress*1. Facilitates the patient/family?s ability to assume responsibility for managing their own health care when appropriate by:a. Promoting return of the patient/family to the highest level of wellness possibleb. Ensuring that the right services are delivered at the right site by the right provider at the right time for the right cost.c. Assisting in the acquisition or verification of financial/payer/insurance information when appropriate.*2. Provides relevant and timely psychosocial assessments of patients and families. Assesses the child?s/family?s needs for case management oversight/intervention. Collaborates with appropriate care coordinators to facilitate continuity of care and optimal health outcomes.*3. Provides crisis intervention as necessary to increase patient/family functioning and facilitate health management.*4. Provides individual, group and family brief counseling to facilitate the resolution of identified psychosocial problems to increase patient/family functioning and facilitate health management.*5. Promotes multidisciplinary collaboration between all members of the health care team to facilitate and coordinate quality patient care in a timely, cost-effective manner.6. Identifies and provides timely clinical social work consultation to interdisciplinary and interagency team regarding psychosocial issues related to children?s health.*7. Identifies, mobilizes and coordinates community and health care resources that support optimal functioning for patients and families.*8. Collaborates with interdisciplinary team to identify, assess, and report suspected child abuse as required by law, with particular attention to risk reduction and prevention.*9. Implements practice strategies, develops, and coordinates programs for risk reduction, health maintenance and illness prevention for individuals, families, and community.10. Documents clinical practice in accordance with JCAHO, Social Work Department, funding agency requirements, and other professional standards.11. Participates in ongoing education related to information technology requirements for the department. Demonstrates competency in PC and mainframe computer applications and software needed to perform job functions.*12. Supports advancement of social work practice through one or more of the following:a. Participates as a speaker/presenter at professional conferenceb. Participates as a speaker/presenter to community groupsc. Promotes staff involvement with professional organizations and activitiesd. Participates on task forces, committees, or advisory groups (department, hospital, community)e. Participates on appropriate Quality Teamsf. Participates in scholarly activities*13. Fulfills programmatic responsibilities specific to the job assignment.14. Maintains professional education requirements as required by state licensure.*15. Identifies opportunities for and provides professional consultation to staff and community professionals regarding psychosocial issues related to children?s health.*16. Contributes to or presents professional educational programs and in-services regarding psychosocial issues in area of specialty that enhance care and services for children.*17. Participates in the development of clinical practice expectations and clinical/social/financial outcome indicators for given population. Analyzes patient and aggregate variances and recommends changes for more effective, efficient, and collaborative delivery systems.QualificationsKNOWLEDGE , SKILLS AND ABILITIES REQUIRED1. Current Social Work license in Ohio required.2. Masters Degree in social work required.3. LISW strongly preferred.4. Two years experience in medical social work preferred.5. Excellent customer service, communication and interpersonal skills required for interaction with all stakeholders.6. Professionally appropriate analytical and organization skills required.MINIMUM PHYSICAL REQUIREMENTSMust be able to see, hear, speak, and perform manual tasks with or without accommodation and care for oneself, with little or no difficulty.
