Part-Time Family Medicine Nurse Practitioner Opportunity
Grace Medical Center/Mobile Van - supporting Population Health Initiatives
LifeBridge Health is seeking a part-time Family Nurse Practitioner to join our Grace Medical Center Ambulatory Services team supporting population health initiatives in Baltimore City and Baltimore County.
LifeBridge Health is a $2B, 13,000 team-member healthcare system that Cares Bravely for over 2.5 million patient encounters annually throughout Maryland. We are comprised of 5 Hospitals, and a network of 130 outpatient practices. Our main hospitals are Sinai Hospital, Northwest Hospital, Carroll Hospital, Levindale Hebrew Geriatric Center and Hospital, and Grace Medical Center. Our LifeBridge Health Medical Group (LBHMG) practices are in Baltimore and in surrounding counties and offer primary and specialty care services to patients within our community.
The qualified Nurse Practitioner will collaborate on the development of our growing our population health services and community partnerships.
The Opportunity
Part-time position: 0.6 FTE (three days/week)
This position will work primarily out of the LBH Mobile Van visiting schools and other community partners in Baltimore and Baltimore County to provide primary care services (i.e., vaccinations, sports physicals, wellness exams, chronic disease management, etc.)
Will be accompanied by a clinical staff member.
Remote access to EMR system, laptop computer and printer
No On-Call responsibilities. No regular weekend or evening hours.
Identify patients needing primary care services and connect them with Grace Medical Center's Ambulatory Services program for continued care.
Requirements
Master's degree and Advanced Practice Nurse, Nurse Practitioner license (APN, APRN, or NP)
Demonstrated computer proficiency. Excellent documentation and organizational skills.
Maryland DEA & CDS license. Current CPR license.
Excellent interpersonal and human relations skills
1 - 2 years' NP experience required
Demonstrated proficiency in direct patient care
Ability to work collaboratively with all members of the health care team, patients, and families
Knowledge of regulatory and accrediting agency standards and requirements (i.e., vaccines for children)
Knowledge of sound nursing and medical practices
