Job Profile: Field Registered Nurse (Non-Clinical) Location: Braintree, MA / Quincy, MA Shift: Day Home Visit: 40 visits/week Job Summary: We are seeking a Registered Nurse (RN) for a non-clinical role with frequent home travel requirements. The incumbent will be responsible for providing healthcare-related services, education, and support in a non-clinical setting, while also traveling to clients' homes as necessary. The role requires a strong foundation in nursing principles, excellent communication skills, and the ability to adapt to various environments and situations. Responsibilities: Client Assessment: Conduct thorough assessments of clients' healthcare needs, including physical, emotional, and social aspects. Healthcare Education: Provide comprehensive education and guidance to clients and their families regarding healthcare management, preventive measures, and lifestyle modifications. Care Planning: Develop individualized care plans based on clients' assessments, goals, and available resources. Coordination of Services: Collaborate with other healthcare professionals, community resources, and support networks to ensure holistic care for clients. Remote Support: Utilize telehealth technologies and communication platforms to deliver remote healthcare services and support to clients. Home Visits: Conduct regular home visits to assess clients' living conditions, adherence to care plans, and overall well-being. Documentation: Maintain accurate and up-to-date documentation of client assessments, care plans, interventions, and outcomes. Travel: Travel to clients' homes as needed to provide in-person support, education, and healthcare services. Health Promotion: Promote health and wellness through advocacy, education, and community outreach initiatives. Quality Improvement: Participate in quality improvement activities to enhance service delivery, client outcomes, and overall program effectiveness. Qualifications: Bachelors degree in nursing (BSN) required. Active Registered Nurse (RN) license in the state of practice. Minimum of 1 year of nursing experience, preferably in a non-clinical or community-based setting. Strong assessment, critical thinking, and problem-solving skills. Excellent communication and interpersonal abilities. Ability to work independently and as part of a multidisciplinary team. Proficiency in utilizing electronic health records (EHR) and telehealth platforms. Valid driver's license and reliable transportation for home visits. Flexibility to travel frequently and work variable hours as needed.
