Role Description

This is a full-time on-site role for a Home Health Nurse located in Northern, VA. The Home Health Nurse will be responsible for providing excellent patient care, including nursing, medication teaching, disease management, wound care, and other necessary home care services. The nurse will collaborate with the interdisciplinary healthcare team to develop and implement care plans, monitor patient progress, and ensure compliance with all applicable regulations and best practices.

Qualifications

• Nursing experience
• Experience in Home Care and Patient Care (preferred)
• Proficiency in Wound Care
• Excellent clinical assessment and critical thinking skills
• Ability to communicate effectively with patients, families, and the healthcare team
• Current nursing license in the Commonwealth of Virginia
• Bachelor's degree in Nursing or related field (preferred)
