OUR DIRECTOR OF NURSING POSITION OUTLINE
The primary purpose of the DIRECTOR OF NURSING position is planning, organizing, developing and directing the day to day nursing functions/services of the Long Term Care units. This position is directly responsible for ensuring quality service delivery and compliance with current federal, state and local standards, guidelines and regulations that govern the Hebrew Home of Greater Washington. In addition, this position collaborates with leadership and the Medical Director; to ensure evidence based practice is integrated into clinical services provided.

ADMINISTRATIVE FUNCTIONS
Demonstrates the ability to, or articulates:
  • Plan, develop, organize, implement, evaluate and direct the nursing service department, as well as its programs and activities, in accordance with current rules, regulations and guidelines that govern the long term care facility.
  • Collaborate with the clinical team and develop, maintain, periodically update, and perform annual review of written policies and procedures that govern day-to-day functions of the nursing department.
  • Collaborate with nursing leadership to maintain and periodically update the Nursing department manuals and the department's service objectives and philosophies.
  • Make written and oral reports/recommendations to the Administrator, as necessary/required, concerning the operation of the department.
  • Establish and monitor process for evaluating the documentation for Medicaid, Medicare, and private pay billing to ensure regulatory compliance and appropriate reimbursement.
  • Attend Bed Management Meetings and facilitate utilization of beds to maximize census and provide for efficient and effective staff utilization.
  • Oversee resident care to ensure continuity of services and adherence to standards of care.
  • Lead proactively by monitoring/trending quality performance activities and performing root cause analysis to develop and implement system/process changes
  • Consult/participate with HR to develop, maintain and periodically update written job descriptions, competencies for staff, and performance evaluations for each level of affected clinical personnel in accordance with the Americans with Disabilities Act, OSHA, and other pertinent laws and regulations.
  • Participate in, develop, implement and maintain an ongoing performance improvement program for the nursing service department.
  • Participate in facility surveys (inspections) made by authorized government and accrediting agencies. Develop, implement and monitor written plan of correction, as indicated.
  • Assist and work cooperatively with the Clinical Development department in developing and implementing appropriate plans of action to correct identified deficiencies and sustained improvements.
  • Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary.
  • Coordinate/oversee care utilization and management function for most cost-effective outcomes.
  • Develop, implement and monitor patient centered initiatives to support quality clinical practices and the overall organizational strategic direction.
  • Work collaboratively with other nursing home departments and Hebrew Home entities, as appropriate, in developing and implementing interdisciplinary clinical programs and practices.
  • Represent the Hebrew Home in community forums, with vendors, and associations/groups, as directed by Senior Management
  • Participate in clinical research initiatives and apply outcomes to develop evidence based practices.
  • Participate in weekly position control meetings.
  • Lead daily stand-up meetings: to ensure clinical issues identified are resolved, to identify potential problems in service delivery and to monitor day to day nursing clinical operations.


PERSONNEL FUNCTIONS
  • Determine staffing needs of the nursing department necessary to meet the total needs of the resident.
  • Recommend the number and level of staff to be employed.
  • Assure the adequacy of staffing patterns and scheduling of staff to meet regulatory requirements, meet residents' needs and address resident service expectations.
  • Assist the HR Director/Department in the recruitment and selection of nursing service staff. (Nurse Managers/Supervisors)
  • Delegate administrative authority, responsibility and accountability to nursing service supervisory staff, as deemed necessary to perform their assigned duties.
  • Assist in standardizing the methods in which tasks will be performed.
  • Review and check competence of staff and make necessary adjustments/corrections as required.
  • Counsel/discipline staff, as necessary.
  • Ensure that departmental disciplinary action is administered fairly and without regard to race, color, creed, national origin, age, sex, religion, handicap or marital status.
  • Terminate employment of staff when necessary, documenting and coordinating such actions with the HR Manager.
  • Assist the HR Director in developing performance evaluation schedules, criteria and annual reviews for the clinical service department (e.g., RNs, LPNs, CNAs, Medication Aides, etc.).
  • Conduct departmental performance evaluations in accordance with the facility's policies and procedures.
  • Create and maintain an atmosphere of warmth, personal interest and positive emphasis as well as a calm environment throughout the facility.
  • Maintain a productive working relationship with other department supervisors and coordinate services to assure that care and services can be performed without interruption.
  • Per policy, review complaints and grievances made by or filed by department staff. Provide reports to appropriate senior management, as required and or requested
  • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility's policies and procedures governing accidents and incidents.
  • Ensure that appropriate adverse personnel actions relative to CNAs' employment criteria are reported to the State Nurse Aide Registry.
  • Ensure that appropriate adverse personnel actions relative to licensed nurses' employment criteria are reported to the State Board of Nursing.
  • Ensure that all clinical service staff follows established departmental policies and procedures, including appropriate dress codes.

LOCATION
This position is based at Smith-Kogod Residence, our post-acute care center, located at 6121 Montrose Road in Rockville, Maryland.

SHIFTS & STATUS
This is an EXEMPT, FULL TIME position on the DAY shift.

MAIN RESPONSIBILITIES
  • Assists the Director of Nursing Services in directing the day-to-day functions of the Nursing Department.
  • Works collaboratively and in coordination with the MDS Manager and PI Manager assigned to their Unit to drive quality and financial performance.
  • Evaluates residents' status and unit activities at the beginning of the shift and assures that appropriate levels of nursing staff are available to provide care. Plans and delegates assignments to nursing staff.
  • Supports staff allocation, recommends staffing levels as appropriate. Shows leadership with the unit staff and helps develop staff.
  • Makes frequent rounds on units to evaluate resident status and staff performance
  • Participate in the development, coordination, maintenance and implementation of the facility's QAPI program for the nursing unit. Monitor falls with injury, moderate and severe pain, PU's, UTI's, catheters, restraints, ADL declines, and other Quality Measures as assigned.
  • Participate in facility surveys (inspections) made by authorized government agencies.
  • Conducts and leads weekly unit meetings with nursing staff, Performance Improvement Manager, and the MDS Manager. Communicates information and changes in facility policies and procedures to staff.
  • Collaborates with the tam to assure resident assessments and care plans are developed, reviewed, and revised as necessary and in compliance with regulatory standards.
  • Assures that physicians' orders are current, appropriate, and communicated to unit nursing personnel.
  • Assures documentation and investigation of incidents and accidents; monitors for accuracy and completeness.
  • Conducts staff performance appraisals. Maintains anecdotal notes and other records pertaining to employee performance.
  • Counsels, educates, and coaches' employees to maintain, improve, and when needed, correct competencies and work habits to support the mission of the home.
  • Makes recommendations to nursing administration regarding hiring, transfer, promotion, and termination of nursing department personnel; Participates in recruitment and retention programs.
  • Conducts environmental rounds; inspects the environment; identifies and reports environmental conditions that are present and a risk and submits requests for correction or repair.
  • Monitors and evaluates infection control practices in collaboration with the facility infection control preventionist.
  • Maintains authorized Unit Budgets as it pertains to staffing, overtime supplies, medical supplies, incontinent supplies, minor equipment purchases and mattress rentals. Makes recommendations to the Director of Nursing. Completes budget variance reports monthly and able to justify being over or under budget
  • Conducts root cause analysis of quality indicators that deviate from acceptable standards; identifies patterns and trends; develops a plan to achieve acceptable standards.
  • Assures that unit practices are consistent with professional nursing standards following HIPAA requirements.
  • Collaborates with ancillary services for efficient, timely and accurate delivery of services and reports (e.g., pharmacy, laboratory, therapies, diagnostics
  • Provides direct patient care when needed.
  • Adheres to professional code of ethics.
  • Ensures that Residents Rights are adhered to at all times.
  • Recognize unusual or emergency situations that warrant intervention
  • Work one off shift every other month to evaluate staff performance. Convey key information to staff, monitors for quality and Standards of Behavior with off shifts

EDUCATION & TRAINING
  • Must possess, as a minimum, a Bachelors degree in Nursing from an accredited college or university.
  • A Masters degree in nursing, health administration or business administration preferred.
  • Must have, as a minimum, three (3) years of experience as a Director of Nursing or equivalent position in a hospital, long term care facility or other related healthcare facility.
