You will participate in the continuum of care for the individuals at your site, and work closely with on-site recreation therapists to ensure adults with all different abilities can access a fun and free lifestyle.This position starts at $60,000 annually, and includes a robust benefits package that sets us above the rest! These duties and responsibilities will be rated on the Annual Performance Review.Participate as directed in the screening of new service requests within the program by means of and agency approved procedure which ensures that all individuals are adequately and appropriately served according to their individual needs and in accordance with existing Board policies and procedure and contractual requirements.Provide case management service through the development of service plans with program participants and their team as the means for implementing supports and developing effective alliances with senior participants.Provide on-going supportive and/or case management functions in accordance with the problems, needs, and the strategies identified within the service plan in order to help the program participants to achieve the stated goals and interests.Coordinate inter-agency planning and service coordination activities as to improve and enhance service continuity and effectiveness including include referrals to other community resources.Provide face to face reviews with the individual and hi/her team on a regular basis. Review the progress made in reaching service goals so that the service plan can be modified as necessary to ensure that the goals and objectives are being achieved. The frequency reviews will be determined by relevant requirements.Additional ResponsibilitiesCoordinate interdisciplinary team meetings held at position location.Support an inclusive environment for participants in senior centers.Complete case management assessment on all program participants entering caseload.Develop individual service plans for program participants as the means for implementing supports and developing effective alliances with program participants, families, and senior center staff.Document all service contacts on a timely basis including face-to-face interviews, collateral and networking contacts, correspondence and maintains the case records in accordance with Fairfax County, ServiceSource's and regulatory standards and requirements.Meet regularly with the immediate supervisor as a means of enhancing professional growth, reviewing, and processing the provision of case management services, and dealing with significant issues.Maintain cords to assure performance contract levels of service have been achieved and/or maintained.Perform other responsibilities as assigned.Qualifications: Education, Experience, and Certification(s )Master's Degree in Social Work, Counseling, or a related field required.Two years' experience working with older adults and/or people with disabilities preferred.Experience in supporting older adults access a continuum of care preferred.Valid driver s license and/or access to reliable transportation to perform work-related travel required. Eligible drivers must have a good motor vehicle record (MVR).Availability and willingness to travel weekly to two or more job sites. Job site locations may vary.Knowledge, Skills, and AbilitiesMust be able to establish and maintain positive working relationships with older adults and individuals with disabilities.Must be committed to a social inclusion model.Availability and willingness to lead multi-disciplinary teamsAvailability and willingness to work with cross agency teams.Proficiency in Microsoft Office skills, and the ability to become familiar with corporate specific programs and software.Excellent verbal and written communication skills.Detail-oriented and able to carry out work with the highest levels of accuracy.Physical DemandsThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
