Overview

PSV has an exciting employment opportunity for an experienced Nurse Care Coordinator to join their dedicated team of healthcare professionals! This specific role will be supporting PSV's Center of Cancer and Blood Disorders Clinic in our Fairfax, VA location. This position is working full-time Monday - Friday. Clinical hours range between 8:00am - 5:00pm.

The role of the Nurse Care Coordinator is to work within the PSV Center for Cancer and Blood Disorders at the Schar Cancer Institute to coordinate care for Pediatric Oncology patients and serves as the clinic liaison for the leukemia/lymphoma, solid tumor, neuro-oncology, and long term follow up teams.

Job Description
  • Coordinates care for the outpatient oncology population in collaboration with the physician and healthcare team.
  • Facilitates all aspects of patient care from diagnosis and throughout the continuum of treatment, including disease specific education, symptom management, and referrals to support services as needed with the help of the multidisciplinary team.
  • Interacts with patients during physician office visits and via phone, assessing patient needs. Triage's patient phone calls and requests appropriately to healthcare team.
  • Documents all interactions, care provided and findings in the medical record.
  • Utilizes order entry for diagnostic testing, prescription refills and other patient instructions requiring provider signature.
  • Acts as a liaison with other departments and institutions to clarify questions related to the care of the patient such as orders and medical records.
  • Provides additional support to the provider including tracking results of laboratory, radiological and pathological studies, triaging results and follow up with patients as needed with results and instructions.
  • Serves as a clinical resource for staff, patients/ caregivers, and other team members.
  • Attends all weekly sub-specialist team meetings and coordinates care on comprehensive clinic days.
  • Coordinates with clinic staff to ensure a positive patient flow, waiting room, and exam room experience.
  • Communicates patient concerns and needs to the healthcare provider.
  • Provides timely responses to patient inquiries and timely completion of assigned tasks.
  • Adheres to and employs appropriate sterile techniques and universal precautions guidelines.
  • Other duties as assigned.

Minimum Qualifications Required
  • Graduation from an accredited school of nursing with a Bachelor of Nursing (BSN)
  • Valid licensure in the state of Virginia as a Registered Nurse (RN) or ability to obtain license in Virginia
  • One (1) year of Pediatric Hematology/Oncology experience
  • Two (2) years of RN work experience
  • Valid BLS/CPR certification
  • Excellent interpersonal and organizational skills; ability to work independently and as a team player
  • Possesses self-initiative, creativity, and motivation
  • Ability to identify and troubleshoot conflicts
  • Exceptional customer service and communication skills
  • Demonstrated computer skills with data entry software
  • Understanding of medical terminology
  • Knowledge of Microsoft Office applications
  • Must have excellent verbal and written communication skills to effectively communicate with both internal and external departmental contacts
  • Strong organizational skills and attention to detail
  • All employees must pass an employment TB, drug, and physical exam
  • Excellent problem solving, analytical, organizational, and oral and written communication skills.

Desired Qualifications
  • CPON or CPHON Certification
  • APHON Chemotherapy / Biotherapy Provider certification
  • At least one (1) year of experience in a pediatric medical office
