Description

Youth For Tomorrow is a 501 (c) (3) and a faith based, non-profit organization, founded in 1986 by Hall of Fame Football Coach Joe Gibbs. The organization is large, diverse, experienced and committed to serving at-risk youth.

Roles & Responsibilities
  • Ensure interagency services are being provided to residents per the ISP and are documented and disseminated to applicable parties in a timely and professional manner.
  • Coordination of all services offered to each resident.
  • Participation in all ITP meetings connected to your caseload.
  • Participation in Treatment Team meetings and ability to effectively present information to groups.
  • Ensure the planning process is oriented toward safe family reunification, preparation for return to the country of origin, transfer to a higher level of care or less restrictive setting, or transition to an alternative living arrangement once legal immigration status has been obtained.
  • Consistently document the provision of services in each client's EHR.
  • Collaborate weekly with the Military Group Facilitator.
  • Coordinate with entities in the field to ensure the ISP is successfully executed for each UAC in care.
  • Ability to work with minimal supervision.
  • Ability to effectively present information to groups.
  • Ability and willingness to work professionally in a team with other personnel, clients, families, etc.
  • Maintain constant communication with the child and family. Meet with each resident twice weekly and communicate with family 1 to 2 times weekly.
  • Maintain complete and updated files for the client in our EHR.
  • Work with Quality Assurance to ensure client files are complete and include detailed case notes, intake, assessment, legal, education, medical, and mental health information.
  • Consult on a regular basis with residential staff, clinicians, case managers and educational staff to ensure resident is receiving optimal services. Review and discuss those areas needing attention.
  • Attend conferences, meetings, and/or court hearings as needed.
  • Maintain resident records and ensure compliance with Interdepartmental Regulations and generally accepted documentation principles.
  • Ensure timely documentation of all required reports.
  • Ensure timely notification of appropriate personnel, parents, and agencies regarding incident reports, medical or dental emergencies, and other Significant Incident Report events.
  • Advocate for the residents and keep staff focused on residents' individual progress and goals.
  • Attend required meetings and participate in formal and informal professional training.
  • Pursue professional development opportunities.
  • Attendance at all training mandated by YFT and respective governing entities, including but not limited to, maintaining current CPR, First Aid certification, Handle with Care certification, and annual TB test/screening.
  • All duties and responsibilities will support meeting the structured program of care outlined in DBHDS and DSS state standards (DBHDS: 12VAC 35-46-800; DSS: 22 VAC 40-151-720).
  • Other duties as assigned by the CEO and Vice President of Programs.


Qualifications

  • Master's Degree in social work, psychology, or counseling; or
  • A baccalaureate degree in social work or psychology with documented fieldwork experience and supervision by the program director or other staff employed by the provider with the same qualifications as required by 22 VAC 40-151-290 D; or
  • A baccalaureate degree and three years of professional experience working with adolescents.
  • Current and maintained CPR, First Aid, and Handle with Care certification.
  • Current and maintained annual TB tests/screenings.
  • Connection to US Military (retired or prior service) or close familial connections.
  • Knowledge of Standards for Interdepartmental Regulation of Psychiatric Children's Residential Facilities and program services for Children and Adolescents.
  • Excellent organizational skills.
  • Ability to communicate well with agencies, parents, and youth.
  • Proficiency in English and prefer proficiency in Spanish as well, but not required.
