Within the inpatient setting, assesses and addresses patient and family needs related to effective and efficient movement through the hospital system and beyond. Performs initial assessment on all patients and begins discharge process from day one of patient's stay. Completes level-of-care assessment to ensure patients are receiving the appropriate level of care and to ensure that the patient transitions through their stay without barriers or delays in care delivery. Maintains expert knowledge of InterQual Level of Care criteria. Works closely with the admitting physicians to ensure appropriate documentation of clinical information necessary to support or determine alternative level of care. Coordinates discharges home and facilitates transfers to external placement facilities.

Position Compensation Range: $67,849.60 - $108,555.20 Annually


MINIMUM REQUIREMENTS

  • Education: Graduate of accredited nursing program. Bachelor of Science in Nursing required within 5 years of hire.
  • Experience: 3 years of relevant experience.
  • Licensure: Licensed to practice as a registered nurse in the Commonwealth of Virginia. Accredited Case Manager (ACM) certification within 3 years of hire.

PHYSICAL DEMANDS

Job requires standing for prolonged periods, frequently traveling, and bending/stooping. Proficient communicative, auditory, and visual skills. Attention to detail and ability to write legibly. Ability to lift/push/pull 20-50 lbs. May be exposed to chemicals, blood/body fluids, and infectious disease.
