Introduction

The Loudoun County Department of Mental Health, Substance Abuse and Developmental Services is seeking a talented and motivated individual to serve as a Support Coordinator in the Support Coordination program. Job Summary

This position is responsible for assessing, coordinating, monitoring, and linking service delivery for children and/or adults with intellectual and developmental disabilities and their families. Experience with the HCBS waivers, CCC Plus Waiver, and Medicaid is preferred. Additionally, knowledge of community-based resources including integrated housing options and employment first principles is desired.
Duties may include assessing eligibility and needs of the individuals, including risk; interviewing individuals and their families; facilitating team meetings, coordinate service delivery among providers, developing and documenting person-centered plans using a strengths-based approach. Assuring quality service provision; maintaining compliance with all State of Virginia Licensure standards, Medicaid standards, and crisis intervention and prevention.
Effective oral and written communication skills, strong organizational and time management skills, and proficiency in utilizing electronic health records and other software programs for documenting service delivery are required. The Support Coordinator is responsible for providing high-quality customer service consistent with the mission, vision, and values of MHSADS.

Minimum Qualifications

Bachelor's degree (Master's degree preferred) in Human Services, Child Development, Early Childhood Special Education, Social Work, Counseling, Psychology, or related field; three (3) years of related experience in assigned program area preferably as a case management, service coordinator, or multi-disciplinary team facilitator with children, youth, adults, and families; or equivalent combination of education and experience.

Master's degree is preferred and the ability to communicate in Spanish using all modes of communication is also preferred.
Job Contingencies and Special Requirements

Employment is contingent upon successful completion of criminal background check, TB test, driving record check, and a valid driver's license in Virginia or regional state of residence.
