Asbury at Bethany Village is now taking applications for either a full-time or part-time Social Worker. Interested candidates should have minimum of BSW with experience in either Long-Term Care living or related work with seniors. Days and hours of work negotiable. Comfort with dementia population necessary, as well as flexibility to assist in all three areas of living on our beautiful campus (skilled, assisted, and residential). Wonderful environment in which to work with potential for professional growth and development.

Schedule & Pay
Job Description
Assists the Director of Social Services in developing, implementing, and maintaining Quality Assurance programs for Social Services.
Develops and maintains professional relationships with BV staff, physicians, residents, families, community resources, and third-party payers.
Completes Admission forms/assessments and documentation as outlined in the Social Service Admission Process
Completes a plan of care for each resident based on their needs. Documents on progress towards care plan goal, and updates POC with significant change and/or quarterly.
Assess residents variouis health needs on an on-going basis. Assess the need for referrals to health services and provides education and support.
Qualifications
Minimum Bachelor's degree in social work and must have current long-term care experience.

- Full Time, 80 hours bi-weekly
- 8am- 4:30pm( flexible on start time)
- Enjoy weekends off
- Pay: $52000-56000
