The Personal Care Aide/Medication Aide assists with patient care throughout nursing areas under the direction of licensed personnel.

* Assists patient with activities of daily living (ADLs) and documents accordingly. Assists patient with feeding and/or meals as assigned and documents accordingly. Assists patient with elimination needs as is appropriate and documents accordingly.

* Administers and accurately records the administration of medications for residents as prescribed by the physician. Determines and records effectiveness of medications.

* Monitors medications to ensure adequate accountability measures are taken when medications are ordered, received into the facility, administered, and at change of shift. Stores medications in a safe and accurate manner.
