Home Care Liaison ( LPN )Position informationTitle: Home Care Liaison -LPNFacility: Fidelity Health CareJob Reports to: Home Care Liaison ManagerGeneral Summary/Responsibilities:The Home Care Liaison is responsible for identifying and coordinating services for home care including durablemedical equipment and infusion. The Home Care Liaison is responsible for the communication of new programs and home care services to hospitals, physicians, staff and patients. The Home Care Liaison is accountable for assisting in the development of strategies to educate ICM staff and physicians regarding clinical programs, as well as conducting an annual review of clinical assessment skills for each RN/LPN.Dimensions:Ensures services are arranged for patients requiring home care services, durable medical equipment and infusion. Responsible for executing strategies identified as priorities by the organization. Actively educates hospital and physician staff in programs and services. Regularly problem solves and communicates opportunities for service enhancement to manager.QualificationsEducationMinimum Level of Education Required: Associate degreeAdditional requirements:Type of degree: AssociatesArea of study or major: RN or LPNPreferred educational qualifications: BSN or LPNPosition specific testing requirement: N/ALicensure/Certification/RegistrationCurrent Ohio RN or LPN License.ExperienceMinimum Level of Experience Required: 1 - 3 years of job-related experiencePrior job title or occupational experience: RN or LPNPrior specific functional responsibilities: N/APreferred experience: One year experience in clinical/ medical setting. Home health experience preferred.Other experience requirements: N/AKnowledge/SkillsSelf directedResults orientedDresses and presents self professionally.Remains current on professional and competitive developments in the home care arena.Strong oral and writing communication skills requiredThe incumbent should be a highly motivated individual with strong speaking, writing and documentation skills.The incumbent should be willing and able to actively engage physicians and staff in educational opportunities regarding Fidelity Health Care and its programs and services.Strong problem solving skills requiredStrong organizational skillsPositive and enthusiastic communicator with Physicians/Hospital Staff and patients.Possesses ability to work in uncomfortable and confrontational situationsActively assists in building relationships with physicians and hospital staffMust be capable of bending, stooping, lifting and carrying up to ten lbs.Must be able to endure repetitive walking throughout the hospital facility.
