Clinical Pharmacist II- Emergency Department
Full time- 7 on- 7 off
Position Summary: Collaborate and consult with physicians, nurses, and other healthcare professionals to ensure safe, efficacious and cost-effective prescribing, dispensing, administration and monitoring of medication in the provision of pharmaceutical care. In so doing, the pharmacist must have current, specialized drug knowledge and demonstrate judgment, communication skills, self-direction and motivation to affect positive interventions in patient outcomes beyond those exhibited by those in a Clinical Pharmacist I position. Participate in educational programs, as necessary, to provide basic and advanced services of the Pharmacy Department. Additionally, perform in the Clinical Pharmacist I role with its accountabilities and job activities pertaining to drug distribution, supervision and non-dispensing functions.
Candidate will provide services including, but not limited to, review of medication history technician work, attend codes and prepare TPA, order review and verification, precepting pharmacy students/residents and serve as a resource to ED providers and nursing.
Qualifications/Requirements:
Education/Knowledge: Seasoned professional knowledge; equivalent to a Master's degree; knowledge in more than one discipline
Education/Knowledge: Completion of a PGY-1 Pharmacy Practice Residency or a minimum of three (3) years of direct patient care experience in hospital setting where knowledge is applied toward pharmacotherapy, resulting in a measurable patient outcome
Experience: 3-5 years
