Available Shifts: 8 am -4:30 pm / 40 hrs/week
Job Description:
The Occupational Therapist (OT) is responsible for an assigned group of patients and is accountable for the oversight and overall management of the occupational therapy services in the home setting.
Ensures appropriate, qualified and timely patient care and documentation in order to meet patient needs and agency financial, operational, and quality goals.
Directs, supervises, evaluates and provides occupational therapy services, ordered by a physician, to patients on an intermittent basis in the home.
Plans and conducts OT programs to restore function with activities of daily living, prevent disability, and help patients reach their maximum level of independence.
May act as a case manager providing management of post-acute services in the home setting including supervision of home health aides.
A licensed occupational therapist's practice is guided by the Occupational Therapy Practice Act and the Board of Occupational Therapy
Essential Functions:
Responsible for the assigned patient caseload, ensuring timely care and documentation to meet the patent needs/services as prescribed in the established plan of care and per MD orders +A2Evaluates the patient's functional ability to perform activities of daily living, potential for achievement, and develops a plan of care in conjunction with the patient's physician.
Designs, develops, and fits patient with orthotic devices and adaptive equipment as needed and instructs all individuals in appropriate therapy methods.
Skills:
Must maintain current state driver's license and auto insurance at all times.
Personal vehicles must be well maintained and in safe driving condition.
1-year experience as a licensed OT in a clinical care setting or home health preferred.
Basic Life Support for Healthcare Provider (BLS HCP) from AHA required.
Education:
Graduation from an accredited school of Occupational Therapy required.
Licensed by the Examining Committee to practice Occupational Therapy in the state required
