About the job:

The Case Management Coordinatorwould work under the direction of the RN Care Managers, Utilization Review Care Manager and the Social Workers. He/She would function with his/her peers and other care providers
for problem solving and facilitating in-patient and post hospitalization care. This person in this position would also help with scheduling follow up appointments for patients, faxing and placing calls to facilities and agencies to assist with discharge planning, faxing information to agencies and facilities to assist with the referral process, notarizing necessary documents and arranging for transportation of patients at discharge as needed
The person in this position also coordinates, oversees, records and transmits information pertinent to the resource management of patients.

Education: Associate's Degree or High School Diploma and equivalent relevant experience required.

Licensure / Certification: Medical assistant or Licensed Practical Nurse (LPN) highly preferred.
Experience: Prior experience in a medical setting required; 5-7 years of customer service, medical assistance or secretarial experience preferred

*This is not a nursing position, it is a support role for the Case Management team.
