The Social Work position at Targeted Case Management performs a fundamental role in the discipline management, recovery mission and compliance with inspection and audit criteria.

The Social Worker will interview clients and family members to obtain detailed social history. They will connect and collaborate with providers to locate housing and other vital community supports. Other responsibilities include admissions tasks such as social assessments, interventions, developing and implementing plan goals/interventions. Discharge planning coordination to support successful re-integration into the community.

The candidate must have experience with documentation, report writing, participating as a professional member of an interdisciplinary team.
