Central Ohio Primary Care Physicians is seeking a Physical Therapist for our Polaris Physical Therapy office located in Westerville, OH. This position will be full-time working first shift hours Monday-Friday.

The Physical Therapist restores patient physical functioning by interpreting physical therapy evaluations and test results and determining physical therapy treatment goals and plans. This includes instructing and assisting patients in performing physical activities and in using assistant and supportive devices. The Physical Therapist is responsible for the overall direction of an effective, ongoing physical therapy treatment plan to facilitate the rehabilitation of patients with physical impairments, and/or functional mobility deficits. This position directs staff and other assigned professionals while maintaining compliance with all applicable laws, regulatory and organizational standards.

Responsibilities:

  • Assess physical therapy patient condition by interpreting evaluations, test results and patient history.
  • Determine course of action and treatment goals/plans in consultation with Physicians and team. Identify and document goals, anticipated progress, and plans to reevaluate throughout treatment plan.
  • Test and measure patient's strength, motor function, sensory perception, functional capacity or circulatory efficiency.
  • Communicate with or instruct patients and caregivers on therapeutic activities or treatment plans. Assist patient in completing tasks and using supportive devices. Keep patients well informed on progress and treatment plans including any updates.
  • Collaborate with Physical Therapist Assistants to get patient services documented in order to keep track of progress and areas needing improved or adjusted.
  • Participate and manage patient discharge and follow up appointments or training opportunities. Facilitate continuing care planning and support to patient.


Requirements:

  • Graduation from an accredited school of physical therapy with a license to practice in the state of Ohio.
  • Established leadership skills with at least 3 years of outpatient experience preferred
  • Excellent communication skills
  • Experience with electronic health record systems- excellent on the job and classroom training will be provided.
  • Must be a team player, but also can work with minimal supervision.
  • Ability to make judgements regarding patient care and clinic operations.
  • Desire to work with patients' ages 14+ of a variety of diagnoses.
  • Prior experience treating vertigo is preferred, but not required.
