We are currently looking for Nurse Practitioners to perform in-home health assessments on Medicare Advantage Members for Lorain County, OH with one of the largest healthcare providers! Please see details below.

• Pay rates are EXTREMELY competitive and paid per assessment.
• Scheduling is based on needs and NP s availability 2 days a week minimum.
• Position can be extremely flexible as a part time position.

Responsibilities

• Conduct Adult/Geriatric Assessments, medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to guidelines and protocols
• Work collaboratively with physicians, case managers, social workers, family members, key caregivers and any appropriate ancillary medical personnel as appropriate
• Collaborate with Primary Care Physician (PCP) on patient education, provide follow-up

Qualifications:

• Have a Minimum of 6 months experience as a Nurse Practitioner
• Will consider new grads
• Master s Degree required OR commensurate experience and satisfactory completion of NP licensure
• Board certified by the AANP, ANCC or the AACCN in an approved specialty
• Board Certifications accepted:
• Acute Care
• Adult Health
• Family Health
• Oncology
• Have a current BLS, ACLS or CPR certification
• Have strong computer skills and familiarity with Employee health/medical record software
