This unique role combines the responsibilities of a Field Nurse and a Case Manager, offering you the opportunity to make a significant impact in the lives of senior patients facing serious illness.

*Position requires only Monday through Friday, 8:30am-5:00pm* No Weekends or On-Call!

*Service Area includes Mechanicsburg, Harrisburg, Hershey and York area

You ll manage and coordinate palliative care, ensuring timely and accurate orders for testing, medications, and referrals. Communication with patients, families, and providers, along with updating treatment plans and addressing concerns, is key. Accurate documentation of health information is essential.

Providing hands-on care during patient visits, including taking vital signs and assisting providers, is crucial. You ll handle medication management, coordinate referrals, therapy orders, and telemedicine appointments, manage prior authorizations, and update care plans as needed. Effective communication and HIPAA compliance are vital.

Palliative Care Registered Nurse (RN) Qualifications:

• Registered nurse RN with current licensure to practice professional nursing in Pennsylvania.
• Graduate of National League for Nursing accredited school of nursing.
• Minimum two years of experience; experience in the area of palliative care, geriatric care, or hospice nursing preferred.
• Self-directed and able to work with minimum supervision.
• Demonstrated ability to prioritize tasks and handle pressure and delicate situations.
• A state driver s license in good standing, and maintain an automobile in good working order.
• High proficiency with computers, Microsoft Office, cellular technology, and EMR required.
• Able to perform the physical tasks that are needed in this role.
