Job description

Thisposition is an on-site position located in North East Portland at a women'ssubstance use residential treatment program. Children between the ages of newborn to 9 years old are also residingon-site with their parent.The

Care Coordinator

provides a widerange of clinical and administrative services to improve client care within awomen's substance use treatment residential setting; to assist in the coordinationand linking of clients to medical and psychiatric services; works to optimizeservice delivery by assisting with and facilitating improved access to clinicalcare across agency programs as part of a strengths-based, consumer-centeredapproach.Location:

This position is based in Northeast Portland.Stabilizewomen who are seeking residential recovery treatment so that they enterand stay in treatment.Helpwomen successfully transition to outpatient care from a residentialtreatment setting.Supportreunification of mothers and children.Support140 women annually through residential program entry with quick access tocomfort medications, prescription refills, mental health care, a PCP, and otherneeded supports.Supportthe women on program exit with connections to methadone clinics, prescriptionneeds, and crisis response.Supportup to 50 children annually with prescription access, psychiatric care, speechtherapy, medical equipment, and other needs.Buildand maintain connections to expand partnerships for methadone clinics and otherresources for consumers on program exit. Must have connections to MATproviders.Usinginformation provided by a variety of sources, identify LifeWorks NWclients with chronic health conditions (e.g. diabetes and SPMI) that areinadequately engaged with physical and oral health providers.Work withinter-disciplinary team to promote coordination of service delivery andprevent duplication of services.Coordinatereferrals with other providers in a timely and professional manner.Collaborate with inter-disciplinary team and community services to ensurethat any diseases, as well as preventative and health promotion needs areaddressed as part of the holistic, whole-person, culturally-sensitiveapproach.Assistidentified individuals with accessing needed physical and oral health careby bringing their situation to the attention of their treatment team andattending clinician.Assist staff workingwith identified individuals with overcoming any barriers impeding accessto physical and oral health care and supporting clients in maintaininghealth.Track referralsand their status on identified metrics to determine project effectiveness.Developrelationships with appropriate staff at medical and dental providers toassist treatment teams and attending practitioners help individuals accessneeded services and coordinate care.Identify caresystems and services to assist in coordination of care with appropriatepartners such as primary care providers, oral health providers or othermedical providers in consultation with team and supervisors.Provide directcare coordination, skills training (i.e. improve functioning andindependence), and case management services.Maintainconfidentiality of records relating to clients' treatment.Meets standardsfor documentation and accuracy of work as defined in the PerformanceEvaluation.Meets standardsas defined in the Performance Evaluation for client care, quality/customerservice, attendance/punctuality and teamwork.Continuallyincrease level of cultural sensitivity, awareness and competency.Participates inregular individual and team meetings with supervisor. Provides written andverbal reports to supervisor regarding cases. Attends informational andclinical meetings, which may include all-agency meetings, team meetings,consultation meetings with psychiatrist, staffings, intra-agencytrainings, etc.Checkse-mail/voice mail daily/throughout the day when working responding tocontacts within 48 hours for routine matter (urgent matters may require amore immediate response)Completesonline training courses as assigned.Travels betweensites or in the community when needed.Qualifications:Can demonstrateexperience and proficiency working with culturally-focused programming forAfrican American women.Required: Theincumbent must demonstrate the competencies as outlined in OAR309-###-#### to become credentialed as a Qualified Mental Health Associate(QMHA).The CADCcredential is preferred, given the high occurrence of substance use/abuse,however not required.Preferred:Clinical experience providing outpatient services or working in communityhealth, primary care is preferred.Preferredaccess to a car, a valid driver's license, proof of current automobileinsurance coverage and verification of safe driving record from DMV.Demonstrateeffective communication skills and ability to provide culturally-competenttreatment services to diverse client populations.Ability toperform essential functions of job without creating a direct threat to thesafety of self or others.LifeWorks NW is a private, non-profit organization providing prevention, mental health, substance use treatment, and related social services to youths, adults, and older adults in over fifteen locations throughout Multnomah, Washington, and Clackamas counties.
