Job description

Our Company BrightSpring Health Services Overview Provide oversight, leadership and support relating to clinical practice and care provision. Consult and collaborate with operations to achieve the highest quality of health care services for consumers and patients. Oversees the delivery of clinical services recommending best practice implementation to and optimal clinical staffing levels as applicable to enhance and improve the quality of health service delivery. Advise and educate personnel on health maintenance and disease prevention. Oversight of the administration of nursing services, survey management, documentation management, incident management, EMR/EHR application, training and education of staff providing direct care and support for individuals we serve. Responsibilities Delivers quality results through clinical excellence and oversight ensuring field clinical staff meet daily/weekly/monthly/quarterly and annual requirements per individual served. Reviews incident report trends ensure follow up as needed. Conduct root cause analysis, identify trends and make recommendations for changes in clinical process, policy and procedures Coordinates the completion of mortality reviews in conjunction with operations at the local level, conducts root cause analysis and makes recommendations for improvement in performance Provides consultation as needed for nursing personnel including but not limited to medication administration, signs and symptoms of illness, clinical operations, IDT meetings, etc. Primary resource for local/regional clinical and nursing leadership to teach, coach, mentor, and train, and precept. Acts as a clinical resource in EMR/EHR systems as necessary Facilitates communication and serves as a liaison between operations and nursing staff and community medical providers, professional consultants to collaborate on care solutions. Conducts regular calls/meetings with assigned states/operations nursing personnel. Works with nursing personnel on survey preparation/ chart reviews/staffing concerns/ED utilization and hospital admission reduction. Provides survey management support/ follow-up/and coordination of corrective actions plans relating to health services and care provision. Collaborates with quality team and operators to ensure appropriate plans of correction are submitted. Monitors reports received from nursing to ensure timely medical follow up Provides consultation and resource for local clinical and care delivery teams for high risk, high volume and problem prone patients/clients, including medically fragile, those with complex care needs, slow to heal or worsening wounds, etc. Assists in the development of plans of care as indicated. Provides feedback to operations management with regard to necessary changes and revisions to care processes and protocols. Surveille for patterns/trends in safety training for staff/risk/workers comp as noted through the critical incident system. Provide nursing Leadership training new initiatives/review services for gaps and educate Provides on-site support as indicated Participates in client case conferences, IPP, IDT, Quarterly reviews, etc as indicated Participates in and approves the selection and hiring nursing leadership positions, provides consultation in direct-care staff hires as needed clinician hires; collaborates/makes recommendations in the hiring process for operations leadership positions. Monitors patterns and trends in state/federal survey citations recommend indicated changes in clinical practice and/or process to drive improvement Build and retain a strong, dedicated team Assists operations management team in fostering positive community relations with external customers as needed. Collaborate with operations on new acquisitions and clinical needs for individuals served. Keeps abreast of healthcare and industry trends, attends industry conferences to grow knowledge and expertise in the diverse fields of practice. Other duties as assigned Qualifications Qualifications: Must have related (5) years nursing management /supervisory experience. 2 years of IDD, community health, post-acute service lines or related experience preferred. Survey management experience required. Education: Degree from an accredited school of nursing. Certificates, Licenses: License as a Registered Nurse (RN), current and in good standing. Valid drivers license from state of residence.
