This is a 50/50 case management role that focuses on Law Enforcement Assisted Diversion (LEAD) and Maryland Readmission Reduction Pilot Program (RRPP).

The Law Enforcement Assisted Diversion (LEAD) Case Manager works in collaboration with the Washington County Sheriff's Department, Washington County Health Department, and Mobile Crisis to provide harm reduction services, peer support, and community case management with the intention to divert potential law enforcement intervention such as incarceration and further disconnection from community services. LEAD participants are adult residents of Washington County, Maryland who are at risk for law enforcement contact due to nonviolent charges including but not limited to trespassing, nuisance reports, drug related offenses (non-distribution), mental health needs, etc. The LEAD Case Manager is compliant with programmatic, state,

and federal regulations, as well as PCS policies and standards of practice

The Readmission Reduction Care Manager (RRCM) delivers point-in-time services for clients being discharged from the hospital for a mental or co-occurring disorder. Key components of the position include patient engagement, assessment, coordination, linkage and follow-up to appropriate providers and services, and ongoing assessment to make necessary adjustments based upon the client's needs. The RRCM will optimize community-based care for each client through an individualized care plan during the 30 to 60-day post-discharge from the hospital. The RRCM is to assist the client as they navigate desired goals and complete actions to improve their mental and medical, social, employment, housing, and other services needed by the client that may impact re-admission. Full compliance with programmatic, state, and federal regulations, PCS policies and standards of practice, and

hospital specified guidelines must be met at all times.

The Maryland Readmission Reduction Pilot Program (RRPP) is a grant-based case management program that works with Meritus hospital to help adult clients transition from inpatient to the community without seeing them readmit to the hospital. Staff complete their own assessments for clients and create individualized recovery plans to help direct what the clients need. Services are initially 60 days in length with potential extensions based off need and/or readmission. Staff are expected weekly contact with all clients and to spend a minimum of 50% of their time with clients. Overall goal of the program is to link clients to community services and prevent inpatient utilization (readmissions or emergency room usage). This may mean spending hours with a client in a given day or seeing them multiple times a week.

  • Due to Meritus requirements, any staff in RRPP must have proof of COVID vaccination. Additional medical screenings are required but are covered as part of the position.
  • Currently, this is a continuation of the original 2-year grant but has only been extended for 1 year (6/30/25).

Job Type: Full-time

Salary: $38,272.00 - $59,800.00 per year (depending on experience and educational attainment)

Requirements

Education and Experience:

Bachelor's degree required, preferred major In Human Services, Psychology, or Social Work.

Minimum 2 years' experience in behavioral health working with adults or 1-year minimum if applicant

has preferred major.

Experience in coordinating and/or delivering services to clients with history of homelessness,

substance abuse, and/or mental health issues.

Strong interpersonal skills

Excellent verbal and written communication

Proof of COVID vaccination. Additional medical screenings are required but are covered as part of the position.

Knowledge, Skills and Abilities:

Works independently while assisting clients and collaborating with co-workers and hospital staff in a

positive manner.

Assesses client needs & concerns while adapting accordingly and reporting pertinent details to direct
