Position Summary: The Integrated Emotional Wellness Therapist works as part of our Integrated Emotional Wellness team to provide high quality mental health and substance use screening and patient support. This person educates patients and other staff about mental health and treatment options, facilitating referrals to other appropriate services as needed. This is a full-time position based out of our Marie Reed Health Center in NW, DC.

Salary- This position starts at $75k and up based on experience. Indeed provides their own estimated salary calculator and is not affiliated with COH's range.

Highlighted Duties and Responsibilities:

  • Supports and closely coordinates mental health care with the patient's primary care provider and, when appropriate, other treating mental health providers.
  • Screens and assesses patients for mental health and substance abuse disorders.
  • Provides patient education about common mental health and substance abuse disorders and available treatment options.
  • Maintains a daily schedule of at least 8 patients seen per day.
  • Provides crisis intervention per the clinician's licensing and COH guidelines, to include addressing suicidal/homicidal ideations.
  • Creates patient safety contracts as appropriate with the patient in crisis and store within patients charts by close of business.
  • Document all direct and indirect care in patient's permanent medical record in accordance with accepted agency, medical, and legal standards;
  • Monitor patients (in person or via telehealth) for changes in clinical symptoms and treatment side effects or complications
  • Provide brief therapy and counseling interventions to clients to support with managing emotional wellness concerns.
  • Completes brief assessments to link to appropriate level of care, provide referral based on need, and coordinating with other providers, telephone contact with patients and their families, and contact with outside agencies.
  • Provides brief interventions using evidence-based techniques such as Behavioral Activation, Motivational Interviewing, or other interventions appropriate for primary care settings.
  • Provides short-term, problem focused therapy for no more than 5 patients, unless approved by the supervisor and for up to 8 sessions per patient as deemed appropriate. Short-term therapy will focus on goal setting, change, and utilizing brief interventions.
  • Completes a treatment plan for all short-term therapy clients by the 2nd scheduled session. Monitors client progress through the course of treatment, reevaluating and adapting the treatment plan at required intervals.
  • Documents all patient encounters, including face to face, telephone, and electronic communications, in the medical record in a timely fashion.
  • Responds to all urgent telephone encounters within 1 business day and non-urgent within 3 business days.
  • Secures patient information and maintains patient confidence by completing and safeguarding medical records and keeping patient information confidential.
  • *Performs other duties as requested by supervisor.
Requirements

Minimum Qualifications:
  • Master's degree in Social Work or related field. For MSW degree holders, the appropriate DC LICSW or LPC license is required.
  • Minimum of two years of experience in a clinical environment required.
  • Thorough knowledge of diagnostic categories, mental health and substance use screening, referral resources, evidence-based treatment modalities, suicide assessment, and therapeutic crisis intervention.
  • Ability to function effectively as a member of an interdisciplinary team and to collaborate well with other disciplines.
  • Experience working with underserved populations preferred.
  • Ability to work flexible hours, including evening and weekends, required.
  • Ability to travel to other sites as needed.

For over 40 years, we have provided healthcare, housing, and supportive services for under-resourced, underserved and people experiencing homelessness in Washington, DC. As a Federally Qualified Health Center, we provide medical, dental, emotional wellness, and care coordination services for the whole family at three locations in DC. Community of Hope also strongly emphasizes maternal and child health, with midwifery practice and the only free-standing birth center in DC.
