Job Description Summary:

Critical Care Experience Preferred

Clinical pharmacists at Grant Medical Center work in a progressive practice model where all pharmacists rotate through central pharmacy and 2-3 clinical areas. Pharmacy services is well integrated into the interdisciplinary teams across the campus and organization. Pharmacists in this position work a mix of day and evening shifts with an every fourth weekend rotation and 2 major holidays per year commitment.

This position is responsible for dispensing and compounding medication, counseling patients on medication and health subjects; interpreting prescriptions, managing patient drug therapy for complications, and performing drug utilization. He/She collaborates with physicians, nurses, and other health-care practitioners to provide safe, high quality, and cost-effective drug therapy. Pharmacists will be required to serve as preceptors for pharmacy students and pharmacy residents in their area of training and expertise.

MINIMUM QUALIFICATIONS

* Bachelor of Science or Doctor of Pharmacy degree from an accredited institution

* Current Ohio pharmacist license

* Previous pharmacy experience

* Basic Life Support Certification-current or obtained during the orientation period (unless greater than 50% of scheduled work hours, as determined by the manager, are completed off-site or not in a patient care setting)

SPECIALIZED KNOWLEDGE

* Pharmacokinetic drug dosing experience

* Drug therapy management

DESIRED ATTRIBUTES

* ASHP accredited residency or equivalent pharmacy practice experience

* Pharmacy Board Certifications or other certifications

* Advanced-level degree in healthcare field

* Collaborative practice experience

* Advanced Cardiac Life Support Certification (may be required to obtain as determined by manager)

Minimum Qualifications:

Required education and credentials:

* BS or Doctor of Pharmacy degree from an accredited institution and a current Ohio pharmacist license

* Completion of a PGY1 Pharmacy Residency or equivalent years of experience

Preferred education and credentials:

* Completion of a PGY2 Critical Care or PGY2 Emergency Medicine or equivalent years of practice experience in those areas

* Board certification

Additional Position Responsibilities:
