Assignment Details:
1. Coordinate, provide for and be able to teach all aspects of the Department of Human Services and Department of Education processes for the Third Party Reimbursement (TPR) Process in collaboration with the TPR Administrative Assistant.
2. Provide licensed school nurse health services and programs within a positive learning environment to preschool and early childhood special education students and youth through health promotion, disease protection and health maintenance birth to 5 years old.
3. Duty days: 8/19/24-6/13/25, position will work 30 hours per week

License Required:
1. Minnesota PreK-12 school nurse license.
2. Masters degree in nursing, public health, or education, preferred.
3. Membership and active participation in professional state and national nursing and educational organizations as School Nurse Organization of Minnesota, National Association of School Nurses, Minnesota Nurses Association, Minnesota Public Health Association.

Job Duties and Requirements:
  1. Provide for compliance with requirements outlined for Parent Consent including being able to explain the elements of the Safeguards, Medical Assistance and Private insurance components to staff and parents along with working with Administrative Assistant in this area.
  1. Provide training for all new special education staff each year in the background and process for TPR through written materials and meetings. Work with Administrative Assistant to train therapists and nurses in tracking on SPEDforms.
  2. Advocate for and suggest strategies to Case Managers to assure that the IEP team considers specific language in the IEP regarding therapist and nursing services, Paraprofessional/Personal Care Assistant (Para/PCA) needs and duties and transportation needs that include details required for TPR.
  3. Provide for timely initiation of Para/PCA tracking for students who qualify for TPR. This includes teaching and collaborating with the Qualified Professional (QP) to develop a Care Plan and do a Care Plan Review with Paraprofessional (identified as training and supervision by the Department of Human Services). This may include starting the care plan and review process at the beginning of the school year because of the workload for QP who are the Licensed School Nurses in the buildings.
  4. Provide training and support to the QP in the ongoing supervision and documentation needs for Para/PCA components to comply with TPR.
  5. Provide training on the background for using ICD10 Codes and the importance of accessing and including these in their tracking process.
  6. Provide information to and collaborate with Human Resources staff and the Administrative Assistant on the requirements for PCA certification and the Excluded Provider List requirements.
  7. Work closely and develop cooperative relationships with Case Managers and Paraprofessionals to advocate for compliance to TPR requirements and early identification of new students who may qualify based on changes in their needs or family situations or new to the district.
  8. Maintain and promote strict confidentiality as described in HIPPA and FERPA for students who are identified as qualifying for TPR.
  9. Respond with care and concern to any parent questions regarding the process Minnetonka Schools uses for all aspects of the TPR process. Refer Parent to Administrative Staff if needed.
  10. Explore and research expanded opportunities for Third Party funding through Mental Health dollars and other private insurance possibilities.
  11. Manage the care of individual students with special health needs through assessment, planning, assurance and evaluation. Interpret health findings for educational staff regarding the impact of health concerns on learning and accommodations required at school. Interpret to medical providers the school related needs of students.
  12. Participate on the IEP (Individual Education Plan) team for students with health/sensory/physical concerns or impairments that may interfere with learning including providing special education related services (direct and/or indirect) in the area of health/physical when needed.
  13. Develop and implement individual health plans (IHP) and emergency health plans (EHP) for students with handicapping health conditions that require accommodation to attend school.
  14. Perform skilled nursing procedures and delegate nursing care procedures when appropriate, including planning, training, monitoring and supervision of delegation using clear written plans for delegates to follow.
  15. Participate in maintaining accurate medical records to assure compliance with state mandates including immunizations, physical examinations and medical conditions using District technology.
  16. Delegate appropriate functions to health paraprofessionals as needed. Provide supervision for their direct and indirect care.
  17. Follow state and county guidelines for reporting contagious conditions and informing families and staff.
  18. Provide planning, intervention, evaluation and reassessment for students with chronic and acute health conditions and/or disabilities in the special or general education setting.
  19. Provide direct professional nursing services such as first aid, illness and emergency care, medication administration, g-tube feedings, catheterizations etc., and health teaching.
  20. Provide mental health support including intervention and/or referral to appropriate school staff or community agencies.
  21. Enhance the educational process through modification or removal of health related barriers to learning and by promoting student wellness.
  22. Perform screening procedures for hearing and vision.
  23. Perform other duties of a similar nature or level.

Specific Responsibilities:
1. Develop Individual Health Plans, Emergency Care Plans and 504 Plans as needed.
2. Collaborate with the special education team to ensure student success. Duties may include; attending special education meetings when appropriate, providing supervision and training to special education paras as needed, obtaining and documenting health assessments.
3. Participate in twice monthly PLC's and follow the other PLC requirements if hired at a 0.75 FTE or greater.
4. Interpret health information and its impact on education planning for students.
5. LSN's participate in growth and evaluation requirements: student engagement plan and principal observations.
6. Participate in community organizations and professional associations that relate to fostering student and families learning, health and development.
7. Attend and participate in local, regional, and state professional organizations and conferences related to student support services and school health programs in order to keep current relative to the field of school health, the district community, and the broader regional and state community.
8. Organize and maintain the emergency response team.

Knowledge and Skills:
1. Evidence of experience as PHN or LSN at elementary and/or secondary level with strong nursing skills preferred.
2. Strong communication skills and interpersonal skills as applied to interaction with coworkers, supervisor, the general public, etc. sufficient to exchange or convey information and to receive work direction.
3. Evidence of skills and experience in working with medically dependent, medically fragile and medically complex students and their families.
4. Demonstrated knowledge in special education process and practice preferred.
