Overview:

Part-Time Outpatient Mental Health Case Manager

20 Hours per week, Potential to become full-time

**Salary Range:$19.00 - $22.50 per hour

RESPONSIBILITIES

Working with clients who have severe and persistent mental health disorders

Assess client needs in their environment and link to appropriate resources

Work with client to formulate and execute individualized service plans

Monitor delivery of services on a continuous basis

Complete Medicaid billing and documentation within the required time frame

Complete chart audits to ensure compliance and quality of services provided

Complete annual required training

QUALIFICATIONS

Candidates will have graduated from an accredited university with a minimum of a Bachelors Degree in Psychology, Social Work, Marriage and Family Therapy, Mental Health Counseling or related Social Services or Behavioral Health field. Candidates must also possess at least one year of experience working with mentally ill adults and children. Candidates must have some knowledge of the Medicaid manual and be willing to be certified by Medicaid within 90 days of hire.

Other requirements:

Must have knowledge and must comply with state and federal statutes which affect the target population

Excellent verbal and written skills a must

Must have a valid Ohio Drivers License and excellent driving record

Must successfully complete all criminal background screenings annually
