Occupational Therapist - Early Childhood-Home Based

Job description

Within the home setting, provide direct implementation, consultation and monitoring of occupational therapy intervention for infants, toddler and preschool aged children across domains and environments. Complete assessments of students to obtain current levels of functioning and need to assist in the development of the IEP or IFSP and related school programming; assessments may include, but are not limited to activities of daily living, mobility, posture, reflexes and fine motor, gross motor, sensory oral motor; assessments may be completed in school, family home and/or community environment.

Performs student assessments; assesses developmental levels of students by formal and informal methods in order to obtain evaluative data; observes and confers with parents and significant others in order to collect and share information relative to the assessment process and to develop the Transdisciplinary Report/Action Plan or Individual Education Plan/Transdisciplinary Report/Action Plan.

Plans, reviews, selects and maintains therapeutic equipment and materials for use in the assigned learning environment; requests, secures and follows up on medical information relative to the occupational therapy plan; maintains the assigned environment in a condition which is conducive for learning and free of health and safety hazards.

Participates as a member of the transdisciplinary team working with families and other professional staff in assessing, developing, implementing, evaluating and modifying outcomes, goals and or care plan for assigned students.

Prepares and maintains written reports and reports as required on, but not limited to, student assessments, program objectives and progress data for regular evaluations and for revision of the Individual Educational Plan/Classroom based Occupational Therapy Program

Provides consultation services with instructors, parents, guardians and/or others to promote generalization of developed skills; may perform delegated nursing tasks and/or procedures

Performs other related duties as assigned
