Description

Maryland Wellness is seeking a Rehabilitation Counselor (RC) to join our Psychiatric Rehabilitation Program (PRP) team in Waldorf! As a Rehabilitation Counselor, you will be responsible for organizing and providing direct support services to clients within their homes and/or the community that will support their journey towards recovery. Requirements

Essential Duties & Responsibilities:
  • Conduct regular in-home or community based client visits to address client goals and provide individualized case management, coaching, and skills training as needed (six times a month for adults, four times a month for minors).
  • Develop and implement Individual Rehabilitation Plans (IRPs) with clients to outline goals for recovery.
  • Identify and connect clients with government and/or community resources as needed.
  • Support client cooperation with treatment recommendations from other providers (therapy, medication management, medical appointments, etc.) and maintain communication with those providers.
  • Provide and/or coordinate transportation to and from scheduled appointments.
  • Ensure timely and accurate completion of all documentation, including (but not limited to): notes, monthly progress summaries, and quarterly IRP reviews.
  • Maintain regular contact with immediate supervisor and engage in both clinical supervision and assigned trainings.
  • Ensure compliance with all agency policies and procedures and state regulations governing the provision of Psychiatric Rehabilitation Services.
  • Communicate regularly and consistently with staff members and its affiliate organizations in regards to scheduling, documentation, and client care.
  • Assist with mental health emergencies as needed.
  • Be a positive representative of the organization by upholding standard messaging and its brand to clients, internal and external providers, referring partners, community affiliates, and any other external third party.

  • Must be highly flexible and able to adapt to change.
  • Self-disciplined, self-starter, and able to work independently with a flexible work schedule.
  • Outstanding interpersonal skills with the ability to effectively build relationships with individuals of all ages, ethnicities, education levels, and work experience.
  • Must be able to operate a vehicle for travel expectations within the community
  • Possesses the ability to strategically, logically, and realistically analyze, evaluate, and develop solutions for a client's rehabilitation needs.
  • Understands and maintains confidentiality and all regulations regarding HIPAA and 42 CFR Part 2.
  • Exhibits professionalism, a friendly demeanor, and is a positive representative of the company over the phone and in person.
  • Excellent written and verbal communication skills.
  • Comfort with computers and technology as needed to input clinical documentation.
  • Strong multi-tasking and organizational skills to manage multiple clients and scheduling.
