Description The Leidos National Security Sector has an opening for a cleared Case Manager who is seeking a rewarding and challenging opportunity supporting medical services for an Intelligence Community (IC) customer in Virginia. This is a position with tremendous opportunity for someone who enjoys supporting an important mission in the National Security arena.Primary ResponsibilitiesCollecting current and historical patient information through conducting initial interviewsCompletion of Physical Capabilities Evaluation Form by obtain medical guidelines from attending physicianCoordinate receipt of all pertinent details necessary for assessment of medical conditionDetermine case direction, establishing specific rehabilitation objectives and goals towards physical and psychological recovery and define associated actionsCoordinate service delivery from external providers and community resourcesImplement medical management through routine physician contact and routine contact with other relevant medical personnel (e.g. physical therapist, occupational therapist, nutritionist, etc.)Coordinate and monitor participation in appropriate rehabilitative programsMonitor and evaluate rehabilitative progress and obtainment of goalPursue job accommodations as appropriate and completed Job Description from the EmployerCoordinate and maintain schedule of daily activitiesFormulate timely case reports, to include presentation of factual information and analysis of suchAdhere to all applicable physician, hospital, facility procedures and protocols during case management activities; build and maintain productive relationships with all professional contactsAs needed, assign any clinical duties to appropriate professional staffAs appropriate, escalate issues to senior personnel and/or management for guidance and resolutionBasic QualificationsAbility to obtain a TS/SCI with polygraph clearance, therefore, US Citizenship is required.Bachelor s degree with 5 years of previous case management experience preferably in military treatment facilities. May consider experience in home health care, and/or mental health facilities.Excellent verbal and communication skills with the ability to communicate concepts to a variety of audiences, both in person and in writingAbility to establish working relationships and effective communication with professional personnel of various backgroundsAbility to communicate clearly, both verbally and in writingPrevious experience in writing factual, clear, and concise reports, to include situational analysisAbility to maintain daily scheduled activities and effective time management skillsExcellent organizational and accuracy skillsPrevious involvement in coordination of medical servicesAbility to conduct thorough medical records reviewAbility to assess medical status and formulate recommendationsDecisive and efficient decision-making skillsPreferred QualificationsDesired to have and maintain a nationally recognized current board-certification as a Certified Case Manager (CCM) from the Commission for Case Management Certification.Original Posting Date:2024-01-30While subject to change based on business needs, Leidos reasonably anticipates that this job requisition will remain open for at least 3 days with an anticipated close date of no earlier than 3 days after the original posting date as listed above.Pay Range:Pay Range $68,900.00 - $124,550.00The Leidos pay range for this job level is a general guideline onlyand not a guarantee of compensation or salary. Additional factors considered in extending an offer include (but are not limited to) responsibilities of the job, education, experience, knowledge, skills, and abilities, as well as internal equity, alignment with market data, applicable bargaining agreement (if any), or other law.
