What youll do:
Ensures compliance with COMAR, MHA, HIPAA, and company regulations
Interviews clients with problems such as personal and family adjustments, finances, employment, food, clothing, housing, and physical and mental impairments to determine nature and degree of problem
Secures information such as medical, psychological, and social factors contributing to client's situation, and evaluates these and client's capacities
Counsels client individually, in family, or in other small groups regarding plans for meeting needs, aids client to mobilize inner capacities, crisis management, and environmental resources to improve social functioning
Helps client to modify attitudes and patterns of behavior by increasing understanding of self, personal problems, and client's part in creating them
Refers clients to community resources and other organizations (i.e. vocational services, educational assistance, etc
Reviews and update individual rehabilitation plan to determine quantity and quality of service provided to client and status of client's case
Submit all required documentation to licensing agencies
Key member of rehabilitation planning and goal production
Secures supplementary information such as employment, criminal records, entitlement records, medical records, or school reports
Coordinate acceptance into program, hospitalizations, follow-ups, and discharges with local agencies and hospitals to evaluate client capabilities and compile necessary documentation for coordination of care
Meet with each client on a monthly basis in their homes to establish and maintain a good working relationship
Responsible for coordination of care with all agencies, doctors, hospitals, families, etc
Identify comprehensive clinical training needs, coordinate, and train staff on a quarterly basis
Conduct and write all assessments and dispositions on new clients in Montgomery County
Set up, conduct, organize, and update treatment team meetings (IRPs or Interim) for RRP clients and Supported Housing clients
Submit follow up forms to the Community Forensics Aftercare monthly and quarterly for clients on conditional releases
Compile, maintain, audit and purge clinical records of consumers in all locations per agency policy and state regulations
Write clinical documentation as needed (authorizations, quality assurance, incident reports, contact notes, etc.)
Ensure client travel is established and maintained
Submit all required maintenance orders for client sites
Attend all mandatory meetings and record minutes
Provide 24-7 on call service for clinical emergencies
Participates in and successfully completes all required trainings
Other duties as assigned
What youll need:
Bachelors Degree with at least three (3) years related experience; OR at least seven (7) years related experience.
Must be able to pass Maryland Board of Nursing CMT.
