Overview

Revival Home Care is currently seeking an LPN to support our Home Health patients. The Home Health LPN works under the supervision of a Registered Nurse. Duties include providing routine care, general observation of client s health status, assists the RN and physician in treatments and communicating with clients and their families/guardians. Responsibilities

Provide direct care to patients in accordance with a comprehensive care plan in the home care settingProvide accurate and timely documentation of patient servicesAssess and provide patient and family/caregiver educationParticipate in coordination of home care servicesParticipate in professional growth and development opportunities and quality assessment activitiesEnsure compliance with all state and federal legal and regulatory requirementsAdditional responsibilities as assigned

Qualifications

Graduate of an accredited practical or vocational nursing program with current licensure to practice in state of operation1 year nursing experience required, 3+ years preferredExperience in community health, hospice or med/surg preferredCommitment to clinical and documentation excellence
