Summary

Case Managers are the lead coordinators of health and wellness services for all clients at Baltimore Safe Haven. In addition, case managers are responsible for assessing, coordinating, and evaluating services for all clients.

Duties and Responsibilities

  • Manages and grows a directory of trans-affirming health and wellness services, including primary care, gender-affirming care, mental health, public benefits, housing, education, and community programs.
  • Facilitates client access to community resources, including locating recreational activities, housing, food, clothing, school programs, vocational opportunities or services, providers to teach life skills, and relevant mental health services.
  • Documents all client encounters and contacts made on behalf of clients; maintains comprehensive client files, including records held for safekeeping on behalf of the client.
  • Occasionally assists with outreach during late night and early morning hours in targeted areas within Baltimore City known to attract sex workers, people who use drugs, or people experiencing homelessness to provide or connect them to services.
  • Other mission-related duties as assigned by management.


Requirements

Required Education and Experience: Minimum three (3) years of related case management experience in public health, mental health, or social work in urban settings and a bachelor's degree in nursing, counseling, sociology, psychology, or case management certificate from an accrediting body in the United States of America. An equivalent combination of education and experience may be considered.

Demonstrated Knowledge, Skills, and Abilities
  • Extensive knowledge of the needs and experiences of TLGBQ people in Baltimore, especially people who are transgender.
  • Critical thinking and problem solving to determine the best plan of action for each client after assessing clients, analyzing notes from health care and social workers
  • Bilingual (English & Spanish)
  • Highly organized, detail-oriented, and proactive in managing several different cases at once.
  • Friendly, compassionate, and confident, and always willing to help.
  • Motivational to encourage clients to follow through on care and transition plans
  • Strong verbal and written communication skills to explain to clients, family members and friends, and professionals the case and care plan and maintain good case records.
  • Computer literacy to maintain and manage case records in case management systems.
  • Current driver's license in good standing (at least 2 years).

Other Information: Incumbent subject to an initial background check and random drug testing.

Must be able to remain stationary 50% of the time. The person in this position may need to constantly move about inside the drop-in center to assist clients, including ascending and descending stairs. In addition, they must operate essential office technology equipment, such as a phone, computer, or tablet. May be required to crouch, kneel and lift to 30lbs. May occasionally be required to work in outdoor weather.
