General Summary

The Care Manager collaborates with members of the healthcare team, community resources, and with the patient/family to promote optimal outcomes for the patient across the continuum of care. The Care Manager provides Care Management activities to ensure an effective, appropriate, individualized and safe plan of care. The position is goal-oriented with a high level of accountability correlated to the continuum of care, quality of care and patient satisfaction.

This role will have a primary focus on Home Health Coordination, and Durable Medical Equipment (DME) Set up. While Home Health Care Coordination and DME set up will be the primary function, there is potential to cover and assist with discharge planning needs as needed within the department.

Reporting Relationships

Reports to the Manager of Care Management.
