Job Overview:

We are seeking a dedicated Nurse Practitioner to join our assertive community treatment (ACT) team. As a Nurse Practitioner, you will provide comprehensive psychiatric evaluation, medication management, and treatment planning. This is a remote (telehealth) hourly, PRN position, where you make your own schedule with clients.

Duties:

- Conduct comprehensive psychiatric evaluation via telehealth

- Develop and implement individualized care plans based on patient needs and medical diagnoses

- Prescribe medications and monitoring, ensuring proper dosage and administration

- Provide education to clients and their families regarding health conditions, treatment options, and preventive measures

- Collaborate with other healthcare professionals to ensure coordinated and comprehensive client care

- Monitor client progress and make necessary adjustments to treatment plans

- Participate in monthly ACT team meetings and treatment planning.

Experience:

- Minimum of 1 year of experience working with individuals with mental illness

- Experience working with adults, 18 years and older

Requirements:

- Valid Nurse Practitioner or Physician Assistant license in Virginia

- Master's degree in Nursing or related field
