Edenbridge PACE at Skyland Town Center is aiming to be the partner of choice for District older adults and their families living in Wards 7 and 8 to allow them to live dignified, meaningful lives and to remain in their own homes for as long as they desire. Six core principles drive our approach to care. These are:

Role in Brief:
The Outreach Coordinator role offers a unique opportunity to continue to build the first PACE (Program of All-Inclusive Care for the Elderly) program in the District of Columbia. As we reach our one-year anniversary, we are looking for ways to expand our reach to target more and more older adults in need of comprehensive medical and supportive services to remain in their own homes for the duration of their lives. In partnership with our coalition of strong partners within the community, the Outreach Coordinator will help Edenbridge PACE at Skyland Town Center foster a transformative impact on a particularly underserved community.

We are looking for an experienced, energetic, mission-driven individual to develop and manage local marketing, community engagement, outreach events, relationship-building, and participant enrollment for the PACE program. At a minimum, the Outreach Coordinator will be responsible for bringing 40 referrals to the program each month, concentrating on individuals who meet the level of care need for the program, who qualify for Medicaid in the District of Columbia, and who reside in Wards 7 & 8.

The Outreach Coordinator is responsible for educating the public, community partners, and referral sources about the Edenbridge PACE at Skyland Town Center program, organizing community outreach and promotional activities, and guiding interested older adults into the intake process. As part of a team including an Enrollment Coordinator, Medicaid Specialist, and department manager, the Outreach Coordinator identifies and cultivates new relationship opportunities that will support the PACE program's goals for enrollment growth. Serving as the primary marketing representative to individuals and organizations who serve frail older adults is an essential component of this position.

This position will report to the Manager of Outreach and Enrollment at Edenbridge PACE at Skyland Town Center.

Main Responsibilities:
  • Identify opportunities to collaborate with community organizations.
  • Provide education to referral sources and the community on PACE services, eligibility criteria, and enrollment processes.
  • Participate in the development of the annual strategic marketing/ community engagement plan to achieve annual enrollment targets.
  • Engage in the enrollment process as necessary to support the enrollment team.
  • Support and assist in the development and execution of community events to foster positive community relationships, including events in the evenings and on weekends as assigned.
  • Assist with development, oversight, implementation, and adherence to policies, procedures, and standards for marketing and community engagement activities.
  • Develop and participate in speaking engagement opportunities for this position and other members of the PACE team.
  • Establish and maintain relationships with community leaders and partners including physicians, hospital discharge planners, post-acute facilities, case managers, social workers, community health clinics, behavioral health providers, and other potential referral sources in the service area.
  • Develop and manage a comprehensive list of housing resources where potential PACE participants live, including housing targeted to older adults as well as multifamily housing properties, and foster and maintain strong relationships with property management and on-site housing staff
  • Collaborate with local Advisory Neighborhood Commissions, AARP chapters, local governmental and quasi-governmental organizations, and others and present to these organizations on a regular basis as requested
  • Investigate and develop a network of religious organizations and identify ways to partner with congregations to meet the needs of the older adults they serve
  • Collaborate with the PACE enrollment team to meet/exceed monthly enrollment targets (10+ new enrollments and a minimum of 40 referrals per month)
  • Assist in the development and administration of the PACE marketing budget, in cooperation with the Manager and Vice President/Executive Director
Qualifications:
Edenbridge PACE at Skyland Town Center is committed to building a diverse and inclusive organization. For our Outreach Coordinator role, we are looking for the following qualifications:
  • Bachelors degree in a related field
  • At least four years of experience working in growth, business development, community partnerships or patient-facing sales in a healthcare context (prior experience in a PACE organization preferred)
  • Experience working independently, including managing own schedule and developing a busy calendar of activities
  • Ability to travel locally within the program services area
  • Familiarity with Medicaid enrollment process preferred
