**Community Support Specialist**

**Department:** 40438 Community Support Services **Location:** Columbia, MD

The Behavioral Health Department of Humanim is currently hiring for a **Community Support Specialist** to join our Community Support Services team! The specialist will provide case management, linkages, coordination and support as needed by consumers. In addition, this position, under the supervision of the Community Supports Program Manager, will conduct initial assessments and reassessments, face-to-face home visits and community visits at a minimum of once every 90 days, develop goals and a care plan with the consumer based on the consumer's strengths and assessment data. The position will demonstrate knowledge of health care, social services, employment, housing, recreational opportunities, and other services and systems available in the community.

**Qualifications:**

* A Bachelor's Degree in a mental health related field and one year relevant experience required, or two years experience with a degree in a non-mental health related field. Master's Degree in Social Work, Psychology, or related field and graduate level licenses preferred.

* Candidate should have experience with behavioral health, rehabilitation and recovery, and case management processes.

* Candidate should demonstrate the ability to assess client's strengths and needs.

* Knowledgeable of the Public Mental Health System in Maryland entitlements procurement, and community resources.

* Valid Maryland driver's license required as well as vehicle.

* Flexibility needed. Ability to be on-call.
