Role and Responsibilities

  • Gather all required Previous Facility Documentation for child's YFT File
  • Conduct calls, interviews, and virtual visits with clients in Spanish or other languages as needed;
  • Assess the potential sponsor's family unit, ability to meet the minor's needs, and educate and prepare the sponsor for accepting the minor into their home;
  • Complete Individualized Needs Assessment and risk assessments for all clients during first virtual visit.
  • Complete virtual home visits to meet with children and their sponsors post-release at 7, 14, and 30 business days;
  • Provide culturally and linguistically appropriate placement services;
  • Provide linkage, resources, referrals, and/or assistance in obtaining services and addressing issues such as safety/placement, legal services, guardianship, education, medical services, and mental health;
  • Work with children and their sponsors to encourage and ensure progress through their individual service plans;
  • Continually assess ongoing changes in behavior and general conditions that could impact the safety of a participant;
  • Refer children and sponsors to a higher level of PRS if necessary
  • Build and maintain relationships with community service providers in relevant service areas;
  • Research all National Service Areas and develop resource list to maintain at YFT that will address all services requirements
  • Participate in regular case management and staff meetings;
  • Maintain accurate records and case files while consistently ensuring the confidentiality of client information which includes the maintenance of all documents in a robust case file;
  • Collect client data based on program reporting requirements and maintain records of measurable outcomes and justification for termination of services;
  • Submit written reports on time as required; and
  • Provide crisis intervention when necessary, including involving Child Protective Services.
  • Maintain complete and updated electronic case files for the child through the designated Electronic Medical Record.
  • Ensure timely documentation of all required reports.
  • Ensure timely notification of appropriate personnel, parents, and agencies regarding incident reports, medical/dental emergencies, and other Notifications of Concern.
  • Attend required meetings and participate in formal and informal professional training.
  • Pursue professional development opportunities.
  • Report to Director of HS/PRS Services and Lead Case Manager regarding quarterly/annual reports and to assist with other tasks.
  • Attendance at all training mandated by YFT and respective governing entity to include but not limited to maintaining current CPR/First Aid certification, Handle with Care certification, and annual TB test/screening.
  • All duties and responsibilities will support meeting the structured program of care outlined in ORR standards HHS-2021-ACF-ORR-ZU-1783; ORR Guide: Children Entering the US Unaccompanied Sec. 2 & Sec. 6
  • Other duties as assigned by the CEO, and Vice President of Residential Services or supervisor.


Qualifications

  • Must be 21 years of age.
  • Proof of HS Diploma or GED.
  • A minimum of one year of experience working with vulnerable populations, preferably immigrants, refugee children and/or victims of human trafficking or children and youth;
  • Fluency in Spanish and English required.
  • Prior knowledge of immigrant community local resources preferred;
  • Excellent organizational, verbal, written, and interpersonal communication skills; Proven ability to collaborate with other staff and service providers;
  • Proficient in Microsoft Office Suite; experience working with client databases helpful;
  • Ability to Prioritize duties in a fast-paced environment;
  • Ability to have a flexible schedule to accommodate occasional after-hours services.
  • Must have a valid U.S. driver's license, valid auto insurance, a clean driving record, and personal transportation
  • Current and maintained annual TB tests/screenings.
  • Ability to communicate well with agencies, parents, and youth.
  • Must be able to travel, including overnight stays, with limited notice. Regional and National travel may be required.
