General Summary of Position

MedStar Health is looking for an Elder Care Advocate to join our team!

The Elder Care Advocate acts as a liaison in the Emergency Department by performing various activities to support older adults seeking care at MedStar Washington Hospital Center. Proactively rounds on patients 65 and older; interacts with the patients socially and clinically to make sure they are safe, comfortable, and dignified, and facilitates information exchange between patients, their families, and members of the healthcare team to positively impact the patient experience. Identifies experience-related issues and tries to resolve them by liaising with the healthcare team and/or other involved departments. Works in partnership with assigned teams and collaborates with Emergency Department and Patient Experience leadership to identify, plan, design and implement improvement strategies.

Primary Duties:

  • Communicates and interacts with patients, families, associates, and others in a compassionate, empathetic, courteous, and respectful manner. Performs daily rounding on patients identified as 65 years of age and older to address pain management, toileting, food and nutrition needs, comfort items such as blankets and pillows, and turning.
  • Communicates with patients and families to mitigate issues that can negatively impact the patient experience by taking ownership to ensure a successful outcome and closure. Distributes comfort kits during rounds as additional resources during their emergency department stay.
  • Tracks and trends elderly patient/family service concerns observed or discovered during rounds. Uses available data sources to identify opportunities for improvement in assigned departments and enhance the patient/family experience as it relates to elder care.
  • Uses patient experience best practices, service improvement, and problem-solving to improve the patient experience. Consults with department leaders in the development of strategies to improve the patient experience for elderly patients in the emergency department; identifies best practice and opportunities for improvement and works collaboratively with leaders and associates.
  • Collaborates with social work to ensure patients and family are provided with appropriate resources to optimize care.


Qualifications:

  • High School Diploma or GED,
  • 1-2 years' recent patient care experience.
  • Certified Nursing Assistant (CNA) or Certified Medical Assistant (CMA).
  • Basic Life Support (BLS).
  • Associate's degree in hospitality, marketing, communications, or business development preferred.
  • 1-2 years' patient/advocacy experience preferred.
  • Emergency Department experience preferred.
