Job description

Provides clinical pharmacy services for patients who are referred by a provider to a medication therapy center specialty clinic or as part of team-based care embedded within a primary care practice. Perform direct patient care under collaborative practice agreements and consultation services. Actively manage medication therapy, complete medication reviews, collaborate with providers to optimize medication therapy, improve adherence, promote wellness, prevent disease, and improve outcomes. This may include recommending or initiating new medication, titrating current medications, monitoring labs, and providing patient education to optimize medication adherence and lifestyle choices. Document all activities appropriately in patient medical record and communicate with other healthcare providers regarding the patient's care.

Collaborate with providers, patients, and caregivers to provide coordinated, comprehensive, high-quality care to patients, including but not limited to disease state management, preventive care, and transition care management.

Provide patient education, promote self-management skills, and encourage patient engagement in their own care.

Confers and consults with and provides information and guidance to various medical, nursing, and other health care professionals concerning drugs and their appropriate use to promote safe and effective drug therapy including but not limited to: dosage schedules, dosage adjustments; therapeutic alternatives, possible adverse drug reactions, drug/drug interactions, drug/food interactions, allergy cross sensitivity, and therapeutic duplication.

Assists in the orientation of new staff and supports student's educational experiences as requested; supports the education of nurses, physicians, and other healthcare professionals about the safe and effective use of medications and new advances and developments related to the clinical use of medications.

Satisfactorily completes required safety training; participates in and/or provides support to hospital committees as agreed upon by committees members and the Director of Pharmacy to help establish drug-related policies and criteria, or to monitor issues related to drugs; participates in departmental and hospital quality improvement activities to promote quality in medication use process; assists department and hospital in maintaining compliance with DNV and other accreditation standards.

Ensures drugs are available to patients at the time needed to avoid delays in therapy and promote positive patient outcomes.

Reviews and assumes responsibility for products prepared by pharmacy technicians and assigns tasks to pharmacy technicians as necessary to meet workload demands.

Interacts with sick, injured, or disabled patients to determine allergy status, medication history, medication taking behavior, health status, and to educate patients regarding drug therapy as necessary.

Assumes duties of the Director of Pharmacy or Ambulatory Pharmacy Manager in his/her absence to manage the daily activities of the department.

Responds to emergencies outside normal working hours.

Requirements

Doctor of Pharmacy Degree from a college or school of pharmacy accredited by the American Council on Pharmaceutical Education preferred (or Bachelor's degree from a college or school of pharmacy accredited by the American Council on Pharmaceutical Education and is able to demonstrate significant clinical pharmacy work related experience and training). Must be eligible for licensure as a pharmacist in Ohio. Residency in hospital pharmacy or clinical specialty residency is desirable. Must maintain licensure as a Registered Pharmacist in State of Ohio; American Heart Association's ACLS certification recommended.
