Position Summary: The Home Visiting Case Manager is accountable directly to the Home Visiting Supervisor. This role will be responsible for providing home visitation services and ensures home visitation activities meet requirements of the DC Health grant and the Parents As Teachers (PAT) evidence based home visiting curriculum. This person must utilize holistic approach to promote positive parent-child interactions and healthy childhood growth and development. This is a full-time position.

Highlighted Duties and Responsibilities:
  • Implement the PAT model to fidelity with families
  • Develops Family Support Plans that are updated at least monthly and signed by clients and support worker, which incorporate goals for both adults and other children in the family if appropriate.
  • Meets with families on case load face to face in the home environment to review progress on Family Support Plan, and to follow-up on tasks and services. Frequency of visits are based on PAT standards.
  • Networks with COH and other community resources for housing, employment, substance abuse treatment, mental health treatment, training, child and family services, etc., and makes referrals as appropriate. Progress on referrals provided, to include follow up, should be documented in the DCRS database
  • Completes required assessments and screenings with families at designated intervals, e.g., ASQ-3, ASQ: SE-2, PHQ-9, Adult Adolescent Parenting Inventory, and the Abusive Behavior Inventory. All completed assessment should be scanned in the DCRS database.
  • Meets weekly with the PAT Program Coordinator for 90-minute reflective supervision sessions. Communicates about client and case management issues in person, via e-mail, or by phone with supervisor as needed outside of regularly scheduled meetings.
  • Works with outside groups, agencies, or individuals to coordinate family activities in addition to attending events as needed.
  • Provides updates on client successes or program outcomes for fundraising and communications purposes as needed.
  • Makes referrals for health services to Community of Hope as needed.


Requirements

Minimum Qualifications:
  • A.A. or B.A in Human Services field (social work, early childhood education, family studies, or psychology) or equivalent professional experience preferred. High school diploma required.
  • BSW and MSW degree holders must have a LSWA/LGSW or appropriate social work license.
  • Experience providing public health education (ex. reproductive health, smoking cessation, child development, and/or parenting) to at risk populations.
  • Demonstrated cultural competence in communicating with underserved, diverse populations.
  • Ability to conduct home visits. Valid driver's license and vehicle required, as well as proof of auto insurance required.

Community of Hope is a mission-driven, innovative, and rapidly growing nonprofit. For over 40 years, we have provided healthcare, housing, and supportive services for under-resourced, underserved and people experiencing homelessness in Washington, DC. As a Federally Qualified Health Center, we provide medical, dental, emotional wellness, and care coordination services for the whole family at three locations in DC. Community of Hope also strongly emphasizes maternal and child health, with midwifery practice and the only free-standing birth center in DC. In 2020, Community of Hope provided about 28,400 medical visits, 7,000 dental visits, and 10,000 behavioral health visits for about 11,000 patients.
